Services Available:
- The clinic provides non-emergency health care to the working uninsured who meet the income eligibility guidelines.
- Your pay stub is your insurance card.
- Treatment is provided by appointment only.
- All prospective patients are screened for eligibility.

- Hypertension
- Diabetes
- Asthma

- Education
- Routine physicals
- Referrals
Services we do NOT provide:
- Dental
- Surgery
- MRI or CT scans
- Screening Colonoscopy
- Sleep studies
We do NOT have any narcotics on site, nor do we prescribe narcotics.
Cancellation policy – If you cannot keep your appointment, please notify clinic staff at least 24 hours before your appointment.
Inclement weather – Please tune into your local TV or radio station for the status of the clinic.
Volunteers – We rely on volunteers to actually provide all medical and support services we offer. If you would like to volunteer for either medical or support services, even a few hours a week, please contact the clinic staff. Click here to download a Volunteer Application.
Eligibility Guidelines (2008):
Our annual income guidelines are:
- Family Size
- Maximum annual family income
Family of 1 $20,800
Family of 2 $28,000
Family of 3 $35,200
Family of 4 $44,400
Family of 5 $49,600
Family of 6 $56,800
Please bring your most recent pay stub and please complete all necessary eligibility, registration and information forms that are required.
Actual eligibility can only be determined once all required documentation is received and reviewed by CCVIM.

Forms can now be downloaded, printed and completed prior to coming to the clinic.
Remember, please do not complete the forms and simply show up at the clinic.
The only thing we can do if you show up without an appointment is to accept your forms and place them in a folder for possible future use.
